SUMMARY:
The Outreach Worker is responsible for providing services for families enrolled in the Early Childhood and Family Learning Foundation’s Mahalia Jackson Center. The Outreach Worker must work with family, center resources and other professionals to promote family literacy and coordinate needed services to Central City residents. The Outreach Worker is responsible for identifying families in need of services, ensure family access to services needed, and maintain communication between Center and enrolled families. The Outreach Worker will serve as an advocate for families in Central City. In addition, the Outreach Worker is responsible for maintaining up-to-date records on enrolled family and ensuring records are in compliance with federal, state, and city regulations.
DUTIES AND RESPONSIBILITIES:
- Identify Central City families in need of social and community services.
- Conduct enrollment for all non-members walk-in to the Center with special effort to identify special needs/disabled family members. This includes assisting families with completion of necessary paperwork for accessing services. Receive calls from families and refer to appropriate services.
- Schedule Family Assessment and work with Family Consultant to educate family on services available to assist with issues identified in the assessment.
- Keep families informed of progress toward family goals.
- Assist families reach self-determined goals and objectives.
- Make referrals as necessary.
- Follow-up with families after all referrals.
- Attend required meetings, conferences, and workshops.
- Maintain open communication with Family Consultant with respect to needed services for children and families.
- Serve as a liaison between the service staff providers, families, programs and community.
- Gather and distribute resources to staff and community as needed.
- Responsible for accurate, complete and timely data collection and submission of participant information for inclusion in Agency Information system (AIS)
- Monthly caseload report and family progress reports. Plan and coordinate social and community service functions.
- Other duties as assigned by the Director of Community and Social Services.
QUALIFICATIONS:
- Must have the ability to communicate effectively with families, social service staff, para-professionals, and other professional support staff.
- Must possess ability to accurately assess situations and refer to the appropriate agencies.
- Must have a working understanding of basic family dynamics and early childhood development, with an emphasis in the area of infants and toddlers.
- Must be able to develop rapport with families and staff.
- Must be knowledgeable of community resources.
- Must be comfortable conducting home visits. Must possess a demonstrated understanding of the dynamics of poverty, oppression, sexism, racism and ageism and other forms of societal and individual prejudice that confront people daily.
- Must have a proven ability to work effectively and respectfully with a broad range of people from diverse cultural, ethnic and social-economic backgrounds.
- Must have skill in engaging and motivating program participants to increase their self-sufficiency and work toward resolution of the challenges they face.
- Must have the ability to advocate on behalf of program participants and must believe that program participants have the capacity to grow and change.
- Must demonstrate a philosophy and value base consistent with the purpose and goals of the Early Childhood and Family Learning Center, utilizing a strengths and empowerment perspective and working towards increasing the capacities of program participants and their families.
- Must possess the ability to work holistically, as part of a team, with professionals, para-professionals, program participants and their families, volunteers and the community at-large towards successful outcomes for program participants.
- Must respect the integrity of each program participant, their right to privacy and confidentiality and their right to participate to their maximum capacity in determining their participation in our programs, including the formation of a case or treatment service plan.
EDUCATION/EXPERIENCE:
- Bachelor Degree in Social Work with RSW credentials or related field of study.
- Two years experience working in a social service setting with children and families.
